Tips for Submitting a Medical Claim

To avoid delays, please make sure your claim is complete and accurate.

Ensure your banking details are correct

Claims refunds can only be processed where valid member banking details are available.

Check your claim and receipt

Ensure your membership number, treatment date, provider details, amount charged, and proof of payment are clearly shown.

Attach all required documents

Include receipts, invoices, prescriptions, referral letters, doctor’s notes, or any supporting documentation where applicable.

Keep copies for your records

Always keep copies of submitted claims and supporting documents for future reference.

  • Your full name and membership number are clearly indicated.
  • The claim form is signed by the member or guardian.
  • The treatment date is shown.
  • The provider stamp and signature are included.
  • The amount charged is clear.
  • Receipts or proof of payment are attached.
  • Referral letters are attached where required.
